Hosted in partnership with the Verona & Sydenham Medical Clinics, there will be another Drive-thru COVID vaccination clinic on Saturday June 12, 2021 from 10am - 4pm at the South Frontenac Public Services Department at 2490 Keeley Rd in Sydenham – by appointment only. This clinic is for first doses only.
If you have not yet received a first dose of any Covid vaccine and are 18 years of age or older, please use the link below to schedule your appointment for a first dose of Moderna.
You do NOT need to be a patient of the Sydenham or Verona clinic to book in. It is open to anyone in our community who meets the eligibility criteria for a first dose. Please note that people under 18 are NOT eligible for Moderna at this time.

After you book your appointment, you should receive an email confirmation with a consent form to review. On the afternoon of your appointment, please wear a mask and a short-sleeved shirt, and bring your signed consent form and your health card. Please do not arrive more than 10-15 before your scheduled appointment. Prior to arriving please review the screening form and if you have symptoms of Covid-19 please cancel your appointment as instructed in your confirmation email.
