Hosted in partnership with the Verona & Sydenham Medical Clinics, there will be a Drive-thru COVID vaccination clinic on Friday May 21st from 2-6pm at the South Frontenac Public Services Department at 2490 Keeley Rd in Sydenham – by appointment only.
If you have not yet received a first dose of any Covid vaccine, and you meet the current eligibility criteria as outlined on the KFLA Public Health website (e.g. age 40 and older, indigenous people, people with high risk health conditions, and people who are in the first or second group of essential workers who cannot work from home such as teachers, grocery store employees, truck drivers, construction workers), please use the link below to schedule your appointment for a first dose of Moderna.
You do NOT need to be a patient of the Sydenham or Verona clinic to book in. It is open to anyone in our community who meets the eligibility criteria for a first dose. Please note that people under 18 are NOT eligible for Moderna at this time.

After you book your appointment, you should receive an email confirmation with a consent form to review. On the afternoon of your appointment, please wear a mask and a short-sleeved shirt, and bring your signed consent form and your health card. Please do not arrive more than 10-15 before your scheduled appointment. Prior to arriving please review the screening form and if you have symptoms of Covid-19 please cancel your appointment as instructed in your confirmation email."
